Summary/Objective:

The Director of Emergency Department’s (ED) primary purpose is to provide direct and indirect provisions of professional nursing care to the patients and accountability for all Team Members functions in the Emergency Department. The Director of Emergency Department will also provide leadership, 24/7 management accountability and assist in promoting evidence-based leadership practices at SCH.  In addition, they will lead initiatives to foster a patient/family centered environment and transform care delivery.

Essential Functions:

  • Coordinate with the medical director, hospital administration and clinicians to assess the need for policies, procedures and protocols relating to the care of trauma patients.
  • Monitor trauma care financial reimbursement issues.
  • Analyze registry data on the trauma patient population; identify trends for strategic planning and performance improvement.
  • Supervise and ensure the accurate collecting, coding, scoring and developing of processes for validation of data entered into the registry.
  • Monitor national and statewide trends in trauma care and participate in state and regional trauma care activities.
  • Evaluates Team Member performance and makes recommendations for merit increases, promotions, and disciplinary action as applicable.
  • Provides fiscal management by developing and recommending department operating budget as instructed and insures compliance. Evaluates and justifies equipment and supply needs. 
  • Directs the day-to-day function of the Emergency Department in accordance with current rules, regulations, and guidelines that govern these areas.
  • Participates in the development, maintenance, and implementation of the hospital’s quality assurance program in the Emergency Department.
  • Participates in multiple organizational-wide and community-based committees as necessary and requested. 
  • Rounds to ensure that ED Team Members and Trauma Coordinator perform their work assignments in accordance with acceptable nursing standards. 
  • Reviews complaints and grievances made or filed by Emergency Department Team Members. 
  • Facilitates EMS and Nursing education, additionally builds relationships with EMS crews.
  • Exhibits flexibility to assist the Emergency Department in times of increased patient load, disaster and/or short staff.

Clinical Education:

  • Plan, coordinate and evaluate trauma-related educational programs for nursing staff.
  • Monitor physician and nurse compliance with the educational requirements of the trauma program.
  • Ensure staff involved in the care of trauma patients meet educational requirements of the trauma program.

Community Outreach:

  • Coordinate and oversee the development and implementation of an injury prevention program. 
  • Direct community trauma education and prevention programs by developing, implementing and evaluating programs for targeted populations in the community related to injury prevention and other topics identified through needs assessment of the community.

  • Bachelors of Science in Nursing (BSN)
  • Current, unencumbered, active license to practice as an RN in Texas.

Summary/Objective: 

Overall responsibility for the business office duties of The Wellness Center. Duties may include typing, filing, patient registration, scanning documents, answering the telephone, scheduling appointments for patients, obtaining prior approval and insurance verification, keeping up with patient orders and signatures as required and faxing/refaxing and follow up phone calls to obtain signatures with physicians. 

Essential Functions:

  1. Answer the telephone using appropriate telephone etiquette.
  2. Greet and check-in patients and inform therapist of their arrival, and screening/temperature checks as necessary. 
  3. Schedule patient’s appointments.
  4. Register new patients to include: demographics, photo ID, insurance card or payment information, brief medical history, appropriate signatures on IPad, completion of appropriate standardized test for the physical therapist, and collect physical therapy prescription.
  5. Verify insurance benefits prior to patient’s visit and inform them of estimated amount owed.
  6. Check charges daily and compare to daily schedule to make sure all charges are submitted/posted.
  7. Record the daily total number of visits.
  8. Prepare monthly summary sheets for # of treatments, evaluations, and other data as requested.
  9. Collect payments, post and take money and receipt to the hospital business office, and fax credit card receipts to front office daily.
  10. Inform medical records when evaluations are signed either by telephone or providing a copy.
  11. Fax initial evaluations, re-assessments, discharge summaries, and certification letters to physicians and follow up on signature to make sure it is obtained.
  12. Keep a log of patients, along with prescription expirations dates, type of insurance, and number of authorized/ prescribed visits.
  13. Scan items for patient’s charts.
  14. Type any letters or other documents as requested by supervisor.
  15. Maintain petty cash box.

Summary/Objective:

The Physical Therapist is a licensed therapist who directs, evaluates and provides Physical Therapy services to patients in the clinic or hospital as prescribed by the attending physician, The Physical Therapist is responsible for maintaining standards of professional and ethical practice in the provision of the full scope of Physical Therapy services, while providing the highest quality healthcare with emphasis on the hospitals vision, mission, and values.

Essential Functions:

  1. Provides treatment as ordered by the physician. If a therapist recommends other treatment, calls to get physician approval, and obtains signature of the recommendation in the form of initial evaluation signature or sends MD an order to sign.
  2. Evaluates disability, injury, or disease process of each individual patient and their function.
  3. Interprets, establishes, plans and implements proper treatment programs in accordance with the physician’s referral. Re-evaluate the program every 10 visits or 30 days (whichever comes first) and makes recommendations to the patient and physician as appropriate (to include continuation of the program or discharge) for submittal.
  4. Contact the physician directly for care as needed.
  5. Works in collaboration with the physician and other members of the health professional team in determining the needs of the patients and in developing their total plan of care.
  6. Documents each visit in patients chart.
  7. Observes, records and reports in the chart and patient’s reaction to the therapy program or any changes in the patient’s condition, and reports to the physician as necessary.
  8. Documents nonattendance in patient’s chart.
  9. Input and check patient charges for billing purposes.
  10. Instructs, teaches and supervises patients, their families and other health team personnel regarding physical therapy procedures as related to the patient physical therapy programs.
  11. Evaluates (as necessary) the home environment and makes appropriate recommendations.
  12. Travel to different Physical Therapy clinics that are operated by Sweeny Community Hospital, to include hospital inpatients.
  13. Participates in staff development activities and attends and participates in hospital forums/meetings.

 Summary/Objective: The program therapist will demonstrate the ability to meet the minimum requirements for the position to include assessments, group therapy, family therapy, treatment planning, discharge planning and coordination with referral sources. Program therapist will develop therapy programs to service patients as appropriate in order to provide adequate care to the patient to improve overall functioning and quality of life.

Essential Functions:

  • The therapist will perform timely processing of inquiry/referral calls and walk in/scheduled comprehensive clinical assessments including the history of presenting illness, family dynamics and supporting psychosocial information to determine eligibility for admission or need for referrals.
  • Program therapist will work collaboratively as a treatment team member to assist medical director and other staff through participation in treatment team meetings to communicate all pertinent information in order to ensure consistent and appropriate care to patients.
  • The therapist provides professional therapeutic counseling by facilitating individual, family, and group therapy sessions.
  • The therapist is assigned a caseload of patients to provide assessments, create master treatment plans to be reviewed monthly, maintaining patient attendance to prescribed treatment, following-up to help ensure the safety of all patients, and discharge planning.
  • The therapist will maintain accurate, complete, and timely documentation on all contacts, sessions, or meetings that are conducted with or on behalf of the patients.
  • Program therapist will engage community education and marketing activities to community agencies in order to provide information about our services and inquire about referrals.

  • At least 6 months’ work experience providing individual, group and/or family therapy, case management and discharge planning in an inpatient or outpatient psychiatric facility; familiarity with ethical and legal issues regarding reporting of abuse and neglect.

  • Prefer at least 2 years of experience working in mental health setting.

 Summary/Objective: The Respiratory Therapist is responsible for providing Cardiopulmonary care services in accordance with specific physician orders, department policies and procedures, and hospital policy.  The Respiratory Therapist administers respiratory care and life support to patients with deficiencies and abnormalities of the cardiopulmonary system, performs diagnostic tests of the cardiovascular and pulmonary system to aid physicians in the diagnosis and treatment of heart and lung disorders.

Essential Functions:

  1. Maintains knowledge of the various methods of oxygen delivery, including the necessary equipment. Remains alert, assesses possible side effects to oxygen delivery.
  2. Administers respiratory therapy treatments, and nebulized medications. Understands the purpose of each and the possible side effects associated with each.
  3. Performs Chest Physio-Therapy involving percussion, postural drainage in order to loosen and mobilize secretions in the lungs and to clear the airway.
  4. Sets-up and monitors mechanical ventilators and performs the critical care duties associated with the procedure.
  5. Monitors patient to ensure adequate ventilation during Cardiopulmonary Resuscitation.
  6. Performs arterial punctures to obtain arterial blood samples and understands the possible complications and contraindications associated with the procedure. The therapist will also perform blood gas analysis.
  7. Performs Pulmonary Function Testing.
  8. Follows departmental equipment maintenance, cleaning, sterilization and packaging procedures as necessary.
  9. Documents all pertinent data in the patient’s medical record following the completion of any service, and reports the condition of the patient to nurse, physician or relief personnel.
  10. Performs EKGs, sends reports to physicians who request them, and files reports with medical records according to department procedures.
  11. Demonstrates knowledge and understanding of the differences in technique and treatment modalities as performed on patients of varying ages, including pediatric, adolescent and geriatric patients.
  12. Demonstrates knowledge and understanding of oxygen concentrations and medication doses as they relate to patients of varying ages, including pediatric, adolescent and geriatric patients.
  13. Manages and operates equipment safely and correctly. Inspects and test respiratory therapy equipment to ensure that it is functioning safely and efficiently.
  14. Maintains Department in a clean, neat and orderly condition.
  15. Treats patients and their families with respect and dignity. Explains the purpose and procedure of treatments, tests and equipment to the patient and family. Coordinates and directs patient care to ensure patient’s needs are met and that hospital policy is followed.
