Counseling Services

The Counseling Services Office at Sweeny Community Hospital provide individualized treatment to those who suffer from emotional or psychiatric disorders through different treatment approaches. Our services our offered in a relaxed, confidential, and caring environment aimed at changing lives through behavioral health services tailored to meet the needs of each patient and our community.

Senior Horizons Program and Counseling Services

With age comes wisdom, experience and unique opportunities for growth and change.  But like all life transitions, navigating through this stage of life can bring about various challenges.  The most common being anxiety, depression, relationship difficulties, financial pressures, loss of identity, adjustment issues, illness, and grief over loved ones.  Senior Horizons is an outpatient counseling program focused specifically on helping our patients learn how to identify, process, and cope with the stressors that they are constantly facing which are having a negative impact on their lives and daily functioning in order to improve their mood, outlook, and overall quality of life. The program provides treatment for approximately four hours a day (9:00 am-2:00 pm) several days a week with transportation being provided to patients in the program from their home to the hospital and returning them back home at the end of the day.

Treatment is covered by Medicare, Medicaid and most private insurance providers. The Senior Horizons Program provides a clinical assessment performed by one of our licensed professionals at no cost to determine the need and appropriateness for treatment.
