Community Health Network Mobile COVID-19 Testing Sites

Community Health Network (CHN) is partnering with Brazoria County, Homeland Preparedness Project, Sweeny Community Hospital and Brazosport College to provide to provide free COVID-19 testing. The hours and locations for the mobile testing site are listed below.

All persons desiring to be tested for COVID-19 must first call the Community Health Center (CHN) Call Center at (281) 824-1480 and complete the CDC recommended pre-screening process. Once the criteria for testing are met, the person will be issued a patient ID number (PID) and directed to the Sweeny Community Hospital Testing Site. Upon arrival at the testing center, all persons will be asked for the PID as verification of pre-screening. All persons that present to the testing site without a PID will be asked to park and call the CHN Call Center to be pre-screened or must return once the screening has been completed. Upon verification of the PID, the person will then proceed to the mobile testing area. CHN’s Call Center staff is prepared to assist with calls or address any question related to testing from Monday–Friday from 7:30 AM to 6 PM at 281-824-1480.
